Job summary

Buckeye Outpatient Clinic in Buckeye, AZ seeks an SMI Case Manager to deliver community-based interventions for adults with serious mental illness. You will assess needs, build service plans, and coordinate care with a multidisciplinary team.

Role requires in-home or community visits, documentation, and collaboration with health plans. Travel is required to support clients in the community, clinics and home settings.

Qualifications

• Bachelor’s degree with one year of full-time behavioral health experience or equivalent clinically-related work.
• Equivalents with five years of healthcare experience accepted.
• Experience with SMI and working with diverse populations preferred.

Responsibilities

• Provide direct services in home, community or office; 1:1 or group interventions.
• Conduct assessments and develop individual service plans.
• Coordinate member care with the care team and external resources.
• Perform crisis stabilization and safety planning as needed.
• Maintain thorough documentation of contacts and progress.
• Travel to home visits and community meetings; monitor treatment responses.
• Collaborate with PCP/Health Plan for integrated care where applicable.
• Transition members to appropriate levels of care after treatment.
• Support transportation and linkage to community services as needed.
