EMPACT-Suicide Prevention Center, part of the La Frontera family, is an industry-leading nonprofit in behavioral health. We are hiring SMI Case Managers at our Phoenix location to complete assessments, develop service plans, and coordinate care across clinical teams and community resources.

Responsibilities include working with Severely Mentally Ill consumers, ensuring service plan implementation, and delivering crisis-aware, confidential support to individuals and families.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.

• Able to be flexible with workload and availability
• Strong internal and external customer service skills
• Ability to exercise independent judgment and maintain appropriate professional boundaries
• Ability to maintain strict confidentiality
• Strong clinical and therapeutic skills and the ability to accurately document events, provide detailed records for members/clients and work with a multidisciplinary team
• Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation
• Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners
• Strong computer skills including experience with Microsoft Office
• A valid AZ Driver's License with no major infractions in past 39 months is required
• Regular access to reliable transportation is required
• Bi-lingual (Spanish) speaking ability a plus

Responsibilities

• As a member of the clinical team, complete assessment and service plans.
• Implement services to consumers identified as Severely Mentally Ill in accordance with ISP.
• Collaborate with clinical team to engage, educate, communicate, and coordinate care with consumers and providers.
• Provide assistance in maintaining, monitoring, and modifying covered behavioral and medical health services.
• Provide brief telephone or face-to-face interactions to maintain or enhance functioning and assist in finding resources.
• Serve as a point of contact to ensure ongoing collaboration including communication of clinical information.
• Ensure the provision of all covered services identified on the service plan and coordinate care across providers.
• Provide outreach and follow-up of services including crisis and missed appointments.
• Provide transportation to the consumer as appropriate.
• Perform all case management functions and sign off on service plans as needed.
• Collaborate with family or significant others to implement effective service plans.
