Valle del Sol in Phoenix is seeking an SMI Case Manager I to deliver case management, care coordination, and psychiatric services for adults referred with Serious Mental Illness. You will assess, plan, and coordinate services across mental and physical health systems, ensuring smooth progress and documentation.

This role supports court-ordered treatment processes, collaborates with clinical teams and legal counsel, and requires reliable transportation and work authorization in the United States.

Qualifications

• Associate degree with 2 years of direct patient contact in healthcare, or a high school diploma with 4 years of healthcare experience.
• Peer Support Certification Preferred.
• Must be authorized to work in the United States.
• Must maintain current BLS certification.

Responsibilities

• Deliver case management, care coordination, and psychiatric services for individuals with Serious Mental Illness.
• Support clients through assessment, treatment planning, and service coordination across mental and physical health systems.
