Job summary

La Frontera EMPACT - Suicide Prevention Center in Phoenix is seeking SMI Case Managers to join the clinical team. You will complete assessments and ISP-based service plans for consumers identified as severely mentally ill, coordinating care with families and providers and documenting progress toward recovery.

The role requires BH experience or the specified education path, strong communication, confidentiality, and crisis management abilities.

Qualifications

• Bachelor’s degree or higher with BH experience requirement.
• Associates degree with BH experience requirement.
• High school diploma with BH experience requirement.

Responsibilities

• Collaborate with clinical team to engage and coordinate care per ISP.
• Maintain and monitor behavioral and medical services coverage.
• Provide brief interactions to sustain functioning and identify resources.
• Coordinate with family, providers, and community supports for service plans.
• Maintain comprehensive clinical records and adjust plans as needed.
