Job summary

A community health organization in Phoenix is seeking a skilled SMI Case Manager to provide direct services and support for adults with Serious Mental Illness. Responsibilities include crisis intervention, assessment, and care coordination with a focus on comprehensive support through in-home and community-based interventions. Qualifications

• Minimum requirement consists of a Bachelor’s degree with one year of full-time behavioral health experience.
• Must have fingerprint clearance issued by the Arizona Department of Public Safety.
• Experience working with multi-cultural and low socio-economic individuals.

• Provide in-home and community-based intervention to adult individuals with Serious Mental Illness.
• Develop service plans based on assessment and diagnostic process.
• Maintain accurate documentation of contact with members and treatment progress.
