Qualifications

• 2 years of direct patient contact in a healthcare setting or 4 years' experience including 1 year of direct patient care.
• Must possess or obtain a valid Fingerprint Clearance Card.
• TB screening and PPD/QuantiFERON/Chest X-ray as applicable upon hire.

Responsibilities

• Deliver case management and care coordination for SMI individuals.
• Manage court-ordered treatment processes and ensure documentation accuracy.
• Coordinate with clinical teams and legal counsel.
