Job summary

EMPACT-SPC in Apache Junction, AZ, is seeking a dedicated SMI Case Manager to assess, plan, and coordinate recovery-focused services for seriously mentally ill individuals. You’ll work with a multidisciplinary team to implement ISP, engage families, and facilitate access to behavioral and medical resources.

The role requires strong communication, documentation, and crisis-capable judgment, plus bilingual Spanish a plus; a sign-on bonus of up to $2000 is available for a limited time.

Qualifications

• Bachelor’s degree or higher with 1 year of behavioral health experience, OR an associate’s degree with 2 years, OR a high school diploma with 4 years.
• Excellent oral and written communication skills.
• Detail oriented with strong organization and confidentiality.
• Ability to work with diverse populations and maintain professional boundaries.

Responsibilities

• Complete assessments and service plans for consumers identified as SMI.
• Coordinate care with family, clinicians, providers, and community resources.
• Maintain and modify covered behavioral and medical services.
• Provide brief, targeted interactions to support functioning and link to resources.
• Document clinical information and keep comprehensive records.
• Ensure continuity of care across providers and levels of care.
• Provide outreach, crisis follow-up, and transportation as appropriate.
• Sign off on service plans and annual updates as needed.
