Job summary

Jewish Family & Children's Service of Arizona is seeking an Adult SMI Case Manager to provide ongoing case management for members designated as Seriously Mentally Ill within our behavioral health program.

The role involves community-based duties, transportation, coordinating services with medical providers, and ensuring follow-through with clients; bilingual Spanish-English preferred; generous benefits and growth opportunities.

Qualifications

• Minimum High School Diploma and experience in behavioral-health field.
• Bachelor's Degree in a human services field preferred.
• Spanish-English bilingual ability preferred; driving and transportation responsibilities require reliable means of travel.

Responsibilities

• Performs duties in the community and provides transportation when necessary.
• Coordinates services for behavioral health and coordination of care with physical health providers.
• Maintains communication with Clinical Team participants and ensures transition-to-adulthood referrals are implemented.
• Understands patient rights regarding treatment access, appeals, and complaint processes.
• Communicates with staff to coordinate and follow through for clients requiring primary care services.
• Performs out-of-office case management and administrative tasks per agency procedures.
