Valle del Sol in Phoenix, AZ, seeks an SMI Case Manager I to deliver case management, care coordination, and psychiatric services for individuals with serious mental illness. You will support ongoing assessments, treatment planning, and service coordination across mental and physical health systems.

The role involves managing court-ordered treatment processes, ensuring accurate documentation, and coordinating with clinical teams and legal counsel.

Qualifications

• Associate degree with 2 years of direct patient contact in healthcare.
• High School diploma with 4 years of healthcare experience (incl. 1 year direct patient care).
• Must be at least 18 years old.
• Must possess or obtain a valid Fingerprint Clearance Card.
• Must maintain current BLS certification.
• TB screening and PPD/QuantiFERON/Chest X-ray as applicable upon hire.
• Must have access to reliable transportation.
• Must be authorized to work in the United States.
• Must be able to pass pre- and continued employment screenings successfully.

Responsibilities
