EMPACT-SPC/La Frontera Arizona in Phoenix, AZ invites qualified candidates for SMI Case Manager roles to assess and plan services for individuals with severe mental illness, coordinating with clinical teams and community resources to support recovery.

The role emphasizes multidisciplinary collaboration, documentation, crisis response, and traveling as needed; sign-on bonus up to $2000 and generous benefits accompany the position.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.

Responsibilities

• Complete assessment and service plans; implement services per ISP.
• Engage, educate, communicate, and coordinate care with the consumer, family, providers, and community resources.
• Maintain, monitor, and modify covered behavioral and medical health services.
• Provide brief telephone or face-to-face interactions to maintain or enhance functioning and connect to resources.
• Coordinate care with medical integration and with family and other providers.
• Ensure provision of all services identified on the service plan and referrals to resources.
• Provide outreach and follow-up including crisis and missed appointments.
• Provide transportation to consumers as appropriate.
• Complete assessments and service planning processes; sign off on service plans.
• Maintain comprehensive clinical records.
• Evaluate treatment effectiveness and modify service plans.
• Adhere to agency policies and assist with QM/QA/Utilization Review.
• Perform additional duties as assigned.
