Job summary

Valle del Sol in Phoenix, AZ is seeking a dedicated SMI Case Manager I to deliver case management, care coordination and psychiatric services for adults and youth with Serious Mental Illness. You will assess and plan services across mental and physical health systems, coordinate court-ordered treatment, and maintain accurate documentation.

Qualified candidates may have an associate degree with healthcare experience or a high school diploma with extensive healthcare experience.

Qualifications

• Must have one of the following education/experience combinations.
• Peer Support Certification Preferred.
• Must be at least 18 years old.
• Must possess or obtain a valid Fingerprint Clearance Card.
• Must maintain current BLS certification
• TB screening and PPD/QuantiFERON/Chest X-ray as applicable upon hire.
• Flu, Hep B, MMR, Tdap, Varicella vaccinations, titers, or signed declination forms
• Must have access to reliable transportation.
• Must be authorized to work in the United States.
• Must pass pre- and continued employment screenings successfully.

Responsibilities

• Deliver case management, care coordination, and psychiatric services for individuals with Serious Mental Illness.
• Support assessment, treatment planning, and service coordination across mental and physical health systems.
• Coordinate court-ordered treatment and ensure accurate documentation and collaboration with clinical teams and legal counsel.
