Job summary

La Frontera EMPACT - Suicide Prevention Center seeks a qualified SMI Case Manager to complete assessments and service plans for Severely Mentally Ill clients. You will implement ISP services, coordinate with families and providers, and document progress toward recovery.

You will ensure continuity of care across providers, maintain comprehensive clinical records, and support crisis intervention, transportation, and follow-up as part of integrated care.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Demonstrated ability to remain calm in crisis situations and work independently as part of a multidisciplinary team.
• Excellent oral and written communication skills and strong organizational abilities.

Responsibilities

• Engage, educate, communicate, and coordinate care with the consumer, family, and providers to implement the ISP.
• Maintain, monitor, and modify covered behavioral and medical health services.
• Provide brief interactions to support functioning and connect to needed resources beyond covered services.
