Job summary

Jewish Family & Children's Service of Arizona seeks an Adult SMI Case Manager to provide ongoing case management for members designated as Seriously Mentally Ill. The role involves community-based activities, coordinating transportation if needed, and ensuring coordinated care with behavioral and physical health providers.

Responsibilities include communicating with the clinical team, facilitating transition-to-adulthood referrals, and handling out-of-office tasks per agency procedures.

Qualifications

• High School Diploma required; Bachelor's in human services preferred.
• Knowledge of mental and behavioral health disorders listed in description.
• Ability to obtain Level 1 Fingerprint Clearance Card.

Responsibilities

• Provides community-based case management and may arrange transportation.
• Coordinates behavioral health services with physical health providers.
• Maintains communication with Clinical Team and transitions to adulthood referrals.
• Handles out-of-office case management and agency procedures in line with Arizona principles.
