Job summary

Southwest Behavioral & Health Services seeks a SMI Case Manager for the Metro Outpatient Clinic. The role involves direct in-home and community-based interventions, crisis stabilization, and care coordination to support adults with serious mental illness.

You will assess, plan, and coordinate services, ensuring documentation and collaboration with the PCP/Health Plan. Travel to communities is required; 3 weeks of PTO and strong benefits accompany the position.

Qualifications

• Bachelor’s degree with 1 year full‑time behavioral health experience.
• Equivalent combination of education and healthcare experience totaling five years acceptable.
• Fingerprint clearance or eligibility to obtain.
• Valid Arizona driver’s license and use of personal vehicle as required.
• Minimum 2 years SMI PNO/IHH experience preferred.

Responsibilities

• Provide in-home and community-based intervention for adults with Serious Mental Illness.
• Assess members per policy and clinical protocols.
• Develop service plans based on assessment and diagnosis.
• Coordinate member care with care team; review efficacy and revise plans.
• Maintain relationships with members and support networks.
