Job summary

EMPACT-SPC, part of the La Frontera family, is seeking a full-time Case Manager at the Apache Junction office to serve the General Mental Health Adult population with flexible scheduling and a focus on community-based care.

You will provide direct services, crisis intervention, transportation and education, coordinate care under the Arizona Practice Model and the 12 Principles, and maintain timely documentation.

Qualifications

• Must meet BHT requirements with either a bachelor's degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Flexible work hours and ability to meet with individuals in their home, community or office.
• Must have Fingerprint clearance card or be eligible to obtain one.
• Must have a valid AZ driver's license with no major infractions in the past three years.
• Must have regular access to reliable transportation.
• Bi-lingual (Spanish)
• Must be at least 21 years of age

Responsibilities

• Provide direct in-home, in-office and community services including crisis intervention, transportation, and education.
• Facilitate, schedule and document Child and Family Team Meetings/Adult Recovery Team Meetings.
• Deliver at least 40 hours of work per week with 30 hours of direct client contact.
• Support families, youth and adults according to the 12 Arizona Principles.
• Maintain client records and produce timely clinical documentation as required.
• Attend regular scheduled weekly supervision and seek additional supervision as needed.
