Job summary

Jewish Family & Children's Service of Arizona is seeking an Adult SMI Case Manager to provide ongoing community-based case management for adults designated as Seriously Mentally Ill. The role emphasizes coordination with behavioral and physical health providers and ensuring smooth access to care.

Responsibilities include arranging transportation, documenting care, and supporting coordination of services with the Clinical Team.

Qualifications

• Minimum high school diploma with relevant experience in behavioral-health field.
• Bachelor's degree in a human services field is preferred.
• Ability to obtain a Level 1 Fingerprint Clearance Card.
• AZ Driver's License and valid auto insurance.
• MVR checks per JFCS driving guidelines.
• Reliable transportation.
• Spanish-English bilingual preferred; Pay differential offered.

Responsibilities

• Provides community-based case management for Seriously Mentally Ill (SMI) members.
• Coordinates services with behavioral and primary health care providers; arranges transportation when needed.
• Maintains communication and coordination with Clinical Team and other staff.
• Supports transition-to-adulthood referrals for clients as applicable.
• Ensures compliance with patient rights related to treatment, appeals, and reporting.
