The Northeast Arc helps people with disabilities become full participants in the community; choosing for themselves how to live, learn, work, socialize and play. The Skills Trainer supports that mission within Northeast Arc's Personal Care Management (PCM) program, directly helping individuals every day.

The Skills Trainer assists with PCA applications, education on employing and paying PCAs, and guidance through Medicaid steps, combining office and community-based work to build lasting

Qualifications

• High School Diploma or equivalent required; BA/BS in Human Service preferred.
• Bilingual a plus; experience with individuals with disabilities preferred.
• Proficiency in Microsoft Office and data entry into a database.

Responsabilités

• Assess personal care needs with nurse/OT to prepare PCA applications.
• Teach clients how to hire, train, supervise, and pay PCAs after approval.
• Provide PCA program education and guidance on all phases of care управление.
• Conduct home visits to assess needs and provide in-person support.
