The Skills Trainer plays a meaningful, community-facing role within Northeast Arc's Personal Care Management (PCM) program, directly supporting that mission every day.

Qualifications

• Strong interpersonal and customer service skills.
• Excellent organizational skills and attention to detail.
• Strong interviewing and assessment skills.
• Excellent written and verbal communication skills.
• Proficient in Microsoft Office applications and entering narrative and data into a database.
• Experience with home visiting and individuals with disabilities preferred.

Responsabilités

• Assess the personal care needs of individuals with a nurse and occupational therapist, and prepare PCA applications for submission.
• Teach individuals how to hire, train, supervise, and pay their own PCAs after approval.
• Provide skills training to consumers with active Prior Authorization under Medicaid curriculum.
• Conduct home visits to assess needs and deliver in-person support.
• Maintain accurate documentation of consumer interactions in the program database.
• Support consumers and surrogates through ongoing case management.
• Collaborate with PCM team to ensure coordinated service delivery.
• Participate in department meetings and trainings related to the Medicaid PCA Program.
