Job summary

The Northeast Arc is seeking a Skills Trainer to support individuals with disabilities within its Personal Care Management (PCM) program. You will assist clients in applying for and managing PCA services, assess needs, and provide hands-on training to help families navigate the process.

In this role you will work both in the office and in community settings, collaborating with nurses, occupational therapists, and administrative staff to ensure coordinated, high-quality care.

Qualifications

• High School Diploma or equivalent required.
• BA/BS in Human Service preferred.
• Experience with disabilities preferred; bilingual a plus.
• Strong interpersonal and communication skills.
• Proficient in Microsoft Office and data entry.

Responsibilities

• Assess personal care needs with a nurse and OT; prepare PCA applications.
• Teach individuals how to hire, train, supervise, and pay PCAs.
• Provide skills training to consumers with active PA and educate on PCA program phases.
• Conduct home visits to assess needs and provide in-person support.
• Maintain accurate documentation in the program database.
• Support consumers and surrogates through ongoing case management.
• Collaborate with PCM team to ensure high-quality service delivery.
• Participate in department meetings and Medicaid PCA trainings.
