Job summary

Qualifications

• Associate degree in social work, human services, or related field from an accredited college or university.
• One year of direct case management experience with residents.
• Ability to develop comprehensive long-term recovery plans using community resources.
• Strong written and oral English communication.

Responsibilities

• Provide comprehensive case management services for emergency shelter residents.
• Conduct resident intake and initial needs assessment.
• Provide linens, toiletries, and hygiene products; replenish as needed.
• Meet with residents to discuss progress and development.
• Prepare and maintain case records and logs; enter information into HCIS.
• Refer residents to counseling resources as needed.
• Collaborate with external case management professionals to meet holistic needs.
