Qualifications

• Associate degree or two years of college coursework in Social Work, Behavioral Science, or a related field.
• Two years’ experience working in a social or public service environment with case management experience.
• Ability to read, write and communicate effectively in English.
• MS Word and Excel proficiency to create and maintain records.
• Ability to interview clients to assess needs and eligibility.
• Ability to work with diverse populations and maintain professionalism.

Responsibilities

• Interview applicants to determine eligibility for program enrollment per guidelines.
• Develop client intake, set goals, and document plan progress.
• Meet with clients regularly to discuss progress and maintain records.
• Plan and facilitate life management classes and workshops.
• Counsel clients and families; refer to appropriate services as needed.
• Prepare and maintain case records and HMIS entries.
• Attend community meetings to educate agencies about the program and refer clients.
• Discharge plans for exiting clients and transfer information as required.
• Support with crisis intervention and group activities as needed.
