Job summary

The Salvation Army in Richmond, VA is seeking a full-time Case Manager to provide comprehensive, structured services to shelter residents. You will develop individualized program plans, assess progress, and connect residents with community resources to address root causes of homelessness.

Role requires an Associate degree in social work or related field and at least one year of direct case management experience; HCIS experience is a plus.

Qualifications

• Associate degree in social work, human services, or related field from an accredited college or university; AND
• One year experience providing direct case management social services including needs assessment and action planning; OR
• Any equivalent combination of training and experience.

Responsibilities

• Conduct resident intake and initial needs assessment; provide linens, toiletries, and hygiene products as needed.
• Meet individually with residents to discuss progress and document records accurately.
• Prepare and maintain case records; enter information into HCIS.
• Refer residents to counseling resources and coordinate with external case management professionals.
