Job summary

The Salvation Army USA Western Territory is seeking a Case Manager to support unsheltered adults in a recovery-focused shelter environment in California. Under the Program Manager, you will manage a caseload, conduct assessments, and develop Individual Transition Plans to guide participants toward housing stability and self-sufficiency.

Responsibilities include coordinating with service providers, documenting progress, and facilitating group activities as scheduled.

Qualifications

• Minimum of two years of social services experience working with marginalized adults.
• CA Driver's License required.
• Bilingual in Spanish preferred.
• First Aid and CPR certification within seven days of employment.

Responsibilities

• Manage an assigned caseload of up to 25 program participants.
• Conduct intake interviews and assessments to identify needs and risks.
• Create Individual Transition Plans with goals and interventions.
• Coordinate with partner organizations and referring agencies.
• Monitor progress and reassess needs to keep plans relevant.
• Educate and empower clients to improve self-sufficiency.
• Respond to and manage crisis situations as needed.
• Maintain client records in the agency database.
• Attend staff meetings and trainings.
