Job summary

The Salvation Army Eastern Territory is seeking a dedicated case management professional to support residents, manage files, and coordinate services with community resources. You will regularly engage with leadership, complete required documentation, and ensure privacy and accuracy of all client records.

You will assess housing barriers, connect residents to services, and help develop individualized housing plans aimed at securing permanent housing in collaboration with community partners.

Qualifications

• Bachelor’s degree in a social service-related field or an Associate’s Degree with 7+ years in social services.
• Knowledge of human services delivery systems and networking.
• Computer proficiency in Word, Excel, PowerPoint, Internet; ability to back up computer data daily.

Responsibilities

• Handle intake, case management and files, statistical reporting, assessment, and service plan updates.
• Meet regularly with Director/Resident Manager on intake and resident progress.
• Coordinate services with community resources and maintain DHS contracted requirements.
• Provide required data to DHS or DHQ and maintain confidentiality of residents’ information.
• Attend trainings and meetings; coordinate services for children.
• Assess housing barriers to determine housing/service needs; work to reduce length of stay.
• Develop and update housing plans; build a timely referral network.
