Integrated Services for Behavioral Health in Ross County, OH is seeking a Care Coordination Supervisor to guide the wraparound care coordination team, ensuring quality services for children and families from diverse backgrounds.

The role requires supervising staff, providing training, and maintaining Medicaid-compliant documentation, while collaborating with community partners to maximize outcomes under OhioRISE ICC/MCC planning processes. A $5,000 hiring bonus is offered.

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology, or related field.
• LSW/LPC preferred.
• Three years of experience in care coordination.
• Previous supervisory experience preferred.
• Experience in behavioral health; multi-system youth or autism/developmental disabilities preferred.
• Knowledge of regulations for behavioral health and related services.
• Excellent communication and organizational skills.
• Experience with Electronic Health Records (EHR).
• Ability to conduct video calls and ensure 100 Mbps internet.
• Willingness to work in home/community settings.

Responsabilités

• Supervise care coordination team activities and meetings.
• Provide training for direct care coordinators.
• Support staff through conferences and case problem analysis.
• Promote teamwork and recognition within the program.
• Ensure completion of care plans, assessments, and reports.
• Coordinate on-call rotation and fidelity monitoring.
• Collaborate with systems and partners to ensure high-quality services.
• Assess needs in crisis situations and coordinate response.
• Maintain documentation meeting Medicaid standards.
• Participate in staff hiring and selection processes.
• All other duties as assigned.
