Job summary

Integrated Services for Behavioral Health in Guernsey/Noble County, OH is seeking a Care Coordination Supervisor to lead the high-fidelity wraparound care coordination for children and families, ensuring culturally competent services and strong community partnerships.

This role offers a $5,000 sign-on bonus, eligibility in the OhioRISE program, and opportunities to train staff, coordinate documentation, and maintain Medicaid standards while advancing quality care.

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology, or related field preferred.
• LSW or LPC credential required for this role.
• Three years of experience coordinating supportive services/care coordination.
• Previous supervisory experience preferred in a behavioral health setting.
• Experience working with multi-system youth, or individuals with autism/developmental disabilities helpful.

Responsibilities

• Supervise and oversee care coordination team members and meetings.
• Provide training for direct care coordinators in home and community settings.
• Support staff through individual and group conferences and problem analysis.
• Promote teamwork and recognition within the team.
• Ensure completion of high-fidelity wraparound training credential and required documentation.
• Coordinate on-call rotation and adherence to planning processes.
• Collaborate with systems and partners to ensure high-quality services for children and families.
• Assist in crisis assessment and response planning.
• Participate in staff recruitment and selection processes.
• Handle other duties as assigned.
