Integrated Services for Behavioral Health in Ross County, OH, is seeking a Care Coordination Supervisor (unlicensed) to lead the care coordination team and ensure adherence to high-fidelity wraparound models. The role offers a $5,000 hiring bonus and requires LIC/LSW preferred with 3 years of care coordination and supervisory experience.

The salary range is $52,609.67 – $63,282.33, based on experience, education, and licensure.

Qualifications

Bachelor’s or Master’s in social work, education, counseling, psychology, or related field.
LSW/LPC preferred.
Three years of coordinated care/care coordination experience.
Supervisory experience preferred.

Responsibilities

Supervise and oversee activities of care coordination team members.
Provide training for direct care coordinators in home and community settings.
Provide routine training, support, and assistance to staff through conferences and analysis of case problems.
Promote an ongoing culture of team member recognition and teamwork.
Complete and maintain high fidelity wraparound training credential.
Ensure completion of care plans, initial assessments, and required documentation.
Coordinate on-call rotation and fidelity review activities.
Collaborate with systems and partners to ensure high-quality services.
Assess needs in crisis and inform response.
Maintain confidentiality and ensure Medicaid compliance.
