Integrated Services for Behavioral Health in Highland County, OH is seeking a Care Coordination Supervisor to oversee a multidisciplinary team delivering wraparound services to children and families. The role ensures fidelity to the wraparound model, develops staff through training, and maintains Medicaid-compliant documentation while collaborating with community partners.

Salary ranges from $52,609.67 to $63,282.33, with a $5,000 hiring bonus based on experience, licensure, and education.

Qualifications

• Bachelor’s/master’s degree in social work, education, counseling, psychology, or another related field preferred.
• LSW/LPC preferred.
• Three years of experience providing coordinated supportive services/care coordination services.
• Previous supervisory experience strongly preferred.

Responsabilités

• Supervise and oversee activities of care coordination team members through various methods, including team meetings/supervision and regular communications.
• Provide training for direct care coordinators in the home and community setting.
• Provide routine training, support, and assistance to staff through individual and group conferences and in improving their skills.
• Promote an ongoing culture of team member recognition and teamwork.
• Complete and maintain high fidelity wraparound training credential.
• Ensure completion of skill and competency-based training to supervise the delivery of ICC and MCC; review and approve all required documentation.
• Coordinate on-call rotation and fidelity monitoring across systems to ensure quality care.
• Collaborate with systems and partners to ensure family voice and choice in services.
• Assess needs in crisis situations and inform appropriate response.
• Maintain confidentiality and Medicaid compliance in documentation.
• Evaluate staff performance and participate in hiring decisions.
• All other duties as assigned.
