Job summary

Canyon Vista Post Acute seeks an RN at the supervisory level to oversee resident assessments, MDS processes, and the plan of care. You will coordinate interdisciplinary teams and ensure timely, compliant documentation for all new admissions.

Responsibilities include leading PDPM meetings, tracking tools, and providing MDS training to new coordinators, with focus on quality outcomes and regulatory compliance.

Qualifications

• Must be in good standing with State Board of Nursing and maintain continuing education/licensing requirements.
• Bachelor of Science in Nursing (BSN) preferred; RN license required.

Responsibilities

• Oversee facility weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools and performance metrics (CSR, PDPM Weekly Tracking).
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions; ensure timely MDS assessments.
• Coordinate MDS documentation and interdisciplinary care planning per regulatory standards.
