Qualifications

• BSN from an accredited nursing program; valid State Board license.
• 3 years of supervisory experience in long-term care preferred.
• Active nursing license and ongoing continuing education requirements.

Responsibilities

• Oversee facility's weekly PDPM meetings for skilled patients.
• Monitor PDPM tools and data analytics for accuracy and utilization.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of MDS assessments including PPS Medicare, quarterly, annual, and significant change.
• Coordinate interdisciplinary participation in MDS and care planning with regulatory compliance.
