Job summary

PACS Group seeks an experienced Nursing Supervisor to oversee facility operations ensuring MDS and PDPM processes meet guidelines. You’ll lead the resident assessment cycle, coordinate interdisciplinary teams, and ensure timely completion of MDS assessments and care plans.

You will train new MDS Coordinators, monitor QA/QASP metrics, and maintain compliant documentation while protecting resident confidentiality. This on-site role requires BSN and strong regulatory knowledge.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• In good standing with the State Board of Nursing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee facility’s weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tool utilization and data reporting.
• Provide PDPM and MDS training to new Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments and PPS Medicare.
• Coordinate interdisciplinary participation for MDS across regulatory time frames.
• Schedule resident care conferences and ensure MDS reviews are completed.
