Job summary

Valley View Post Acute is seeking a RN with leadership skills to oversee the resident assessment process, including MDS and PDPM activities. You will ensure timely MDS submissions, coordinate interdisciplinary care, and supervise staff involved in assessments.

The role requires a BSN or equivalent, RN licensure, and prior supervision in a long-term care setting. Strong data analysis and familiarity with MDS platforms preferred.

Qualifications

• Registered nurse with BSN preferred.
• Must be in good standing with state board and maintain CE requirements.
• Minimum 3 years of supervisory experience; long-term care experience preferred.

Responsibilities

• Oversee facility to ensure MDS assessments are completed per guidelines.
• Lead weekly PDPM meetings for skilled patients and monitor PDPM tools.
• Provide PDPM and MDS training to new MDS Coordinators.
