Job summary

Canyon Vista Post Acute is seeking a RN Nursing Supervisor to oversee the facility's resident assessment process, including MDS and plan of care development. You will coordinate PDPM-related meetings, monitor utilization of PDPM tools, and guide the team to maintain compliant documentation.

The role requires at least a BSN, active State Board of Nursing, and 3 years of supervisory experience, preferably in long-term care. Strong MS Office and PCC knowledge are also expected.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficient in Microsoft Suite products; PCC knowledge is preferred.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines including MDS and resident plan of care.
• Provide oversight on weekly PDPM meetings for skilled patients.
• Track PDPM tools and data; provide analytics and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate interdisciplinary participation for MDS completion within regulatory time frames.
• Maintain an accurate schedule of all MDS assessments and reference dates.
• Data entry and electronic submission of MDS assessments.
