Job summary

PACS seeks a facility-based Nursing Supervisor to oversee the resident assessment process, including MDS assessment, and the development of the resident plan of care. You will ensure assessments are completed per guidelines and coordinate interdisciplinary activities to optimize outcomes.

The role requires leading PDPM and MDS activities, supervising weekly PDPM meetings, and training new MDS Coordinators while maintaining regulatory standards and high-quality documentation.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred; must be in good standing with the State Board of Nursing and maintain continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee the facility's weekly PDPM meetings for skilled patients.
• Ensure timely and accurate completion of MDS assessments and PPS Medicare processes.
• Coordinate interdisciplinary involvement in MDS for new admissions and ensure regulatory time frames are met.
• Provide PDPM and MDS training to new MDS Coordinators; monitor QM and QASP performance.
