Job summary

Antelope Valley Care Center is seeking a seasoned MDS Nurse (RN) to oversee the resident assessment process, including MDS assessments and the plan of care. The role focuses on PDPM coordination, staff training, and ensuring regulatory compliance in a long-term care setting.

The ideal candidate holds a BSN, is in good standing with the state nursing board, and has supervisory experience, preferably in long-term care. Excellent data skills and collaboration across disciplines are essential.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee the facility's resident assessment process, including MDS assessments and the plan of care.
• Coordinate PDPM tools and attend weekly PDPM meetings for skilled patients.
• Monitor QM and QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Maintain reference dates and data entry for MDS submissions.
• Coordinate interdisciplinary participation in completing the MDS within regulatory time frames.
