Job summary

Antelope Valley Care Center is seeking an RN with strong MDS/PDPM expertise to oversee the resident assessment process and ensure compliance with federal and state guidelines. The role includes supervising the MDS team, coordinating care plans, and ensuring timely data entry for MDS submissions.

The ideal candidate will have BSN preferred, active RN license, and at least 3 years of supervisory experience in long-term care, with proficiency in Microsoft Office and PCC tools.

Qualifications

• BSN preferred and must be in good standing with the State Board of Nursing.
• 3 years of supervisory experience desired.
• Knowledge of PDPM/MDS processes and LTC regulations.

Responsibilities

• Oversee PDPM/MDS processes and weekly meetings for skilled residents.
• Coordinate and supervise data entry and electronic submission of MDS assessments.
• Lead interdisciplinary care planning and ensure timely, compliant documentation.
• Conduct resident care conferences and monitor level of care changes.
• Provide PDPM/MDS training to new coordinators and ensure regulatory compliance.
