Job summary

Avcarecenter seeks an experienced MDS Nurse (RN) to lead the resident assessment process at Antelope Valley Care Center. This role oversees MDS and care plans, ensures timely assessments, and coordinates interdisciplinary participation to meet regulatory standards.

The ideal candidate has a BSN, supervisory background in long-term care, and strong PDPM/MDS knowledge to guide decisions and improve resident outcomes.

Qualifications

• Bachelor of Science in Nursing preferred.
• 3+ years supervisory experience in long-term care or equivalent.
• Strong knowledge of MDS, PDPM, and care planning.

Responsibilities

• Oversee facility MDS assessments and resident care plan development.
• Lead weekly PDPM meetings and track utilization and outcomes.
• Coordinate interdisciplinary MDS completion within regulatory timeframes.
• Provide PDPM and MDS training to new coordinators and staff.
