Job summary

Sage Creek Post-Acute in Las Vegas, NV seeks an experienced Nursing Supervisor to oversee the facility's resident assessment process and related MDS activities, ensuring accuracy and regulatory compliance.

You will lead weekly PDPM meetings, monitor PDPM tools, provide PDPM and MDS training, coordinate interdisciplinary care planning, and ensure timely documentation across the team, with emphasis on quality outcomes.

Qualifications

• Bachelor of Science in Nursing from an accredited school.
• Must be in good standing with the State Board of Nursing and maintain CE/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficient in Microsoft Office products; PCC experience preferred.

Responsibilities

• Oversee the facility's weekly PDPM meetings for skilled patients.
• Track and monitor utilization of PDPM tools and data reporting.
• Monitor QM and QASP performance with data analysis and clinical recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
