Job summary

Qualifications

• Accredited nursing program with BSN preferred.
• In good standing with the State Board of Nursing and ongoing education/licensing.
• Minimum 3 years supervisory experience in long-term care.

Responsibilities

• Oversee resident assessment process and MDS completion.
• Coordinate interdisciplinary participation for the MDS and PDPM processes.
• Train new MDS Coordinators on PDPM and MDS procedures.
• Ensure timely, accurate data entry and electronic submission of MDS.
• Maintain and update care plans, level of care determinations, and documentation.
• Monitor QM/QASP performance and provide data-driven recommendations.
