Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate care for individuals with complex needs across the United States. The LCM partners with healthcare providers, social services, and families to ensure person-centered, timely, and cost-effective care.

This role covers in-home and telehealth services, development of care plans, documentation, and ongoing advocacy. The ideal candidate has 2+ years in care management, strong communication, and knowledge of social determinants of health.

Qualifications

• Minimum 2+ years as a care manager, navigator, or community health worker.
• Willing to work Monday–Friday 8:30am–5:00pm Pacific Time with potential evenings/weekends.
• Working knowledge of government and community resources for social determinants of health.
• General computer skills; familiarity with Google Workspace, MS Office, and the internet.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative, memory, and housing.
• Develop and update client care plans and goals.
• Provide services where the member resides (office-based, telehealth, or field-based).
• Connect clients to social services and supports.
• Advocate with healthcare professionals using evidence-based practices (e.g., Motivational Interviewing, Harm Reduction, Trauma‑Informed Care).
• Conduct outreach and log activities in the CRM.
• Complete all documentation and outcome measures within care plans.
• Maintain patient health records in the EMR and other systems.
• Attend training as assigned.
