Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate care for clients, partnering with providers, social services, and caregivers to ensure seamless, person-centered support. This remote role reports to ECM leadership and drives comprehensive planning and access to services.

The LCM collaborates across care teams to promote wellness, resilience, and member empowerment while maximizing benefits and cost-effective care pathways.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for evenings/weekends.
• Working knowledge of government and community resources related to social determinants of health.
• Clean driving record, valid driver's license, and reliable transportation.
• Excellent oral and written communication skills.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD and more.
• Oversee development of client care plans and goals.
• Offer services where the member resides (office, telehealth, or field).
• Connect clients to social services and supports.
• Advocate for clients with healthcare professionals.
• Utilize Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and log activity in the CRM system.
• Evaluate progress and update SMART goals.
• Provide mental health promotion.
• Arrange transportation (e.g., ACCESS).
• Complete documentation and outcome measures on time.
• Maintain EMR and other patient records.
• Complete monthly program compliance reporting.
• Attend training as assigned.
