Vynca Inc. is seeking a Lead Care Manager to coordinate comprehensive, person-centered care for individuals with complex needs.

This role partners with healthcare providers, social services, and family caregivers to maximize member benefits, promote wellness, and ensure timely access to care. The LCM will oversee care plans, coordinate benefits, and advocate for members across office-based, telehealth, and field-based settings, reporting to ECM leadership.

Qualifications

• 2+ years experience in care management or community health roles.
• Willing to work Monday–Friday 8:30am–5:00pm Pacific Time, with possible evenings/weekends.
• Working knowledge of government and community resources related to social determinants of health.
• General computer skills; proficient with Google Workspace, MS Office, and internet.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, and social supports.
• Develop and monitor client care plans and goals.
• Provide services where the member resides, including office, telehealth, or field-based.
• Link clients to social services and supports as needed.
• Advocate with health care professionals using Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and log activity in the CRM system.
• Complete documentation and outcome measures within care plans timelines.
• Maintain up-to-date patient health records in the EMR and other systems.
• Attend required training.
