Qualifikationen

• 2+ years experience in care management, care navigator, or community health worker roles.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with potential evenings/weekends.
• Knowledge of government and community resources related to social determinants of health.
• Clean driving record and reliable transportation.
• Excellent oral and written communication skills.
• Strong interpersonal skills and teamwork abilities.
• General computer skills with Google Workspace, MS Office and internet.
• Bilingual English/Spanish preferred.

Aufgaben

• Assess member needs across physical health, mental health, SUD, oral health, palliative, memory and trauma-informed care, social supports, housing, and connections to community services.
• Oversee development of client care plans and SMART goals.
• Provide services where the member resides, including office-based, telehealth, or field settings.
• Connect clients to social services and supports as needed.
• Advocate on behalf of the client with health care professionals.
• Utilize evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and engagement to link clients to the ECM program and log activity in CRM.
• Evaluate progress and update SMART goals.
• Promote mental health.
• Arrange transportation (e.g., ACCESS) as needed.
• Complete all documentation and outcome measures within care plan timeframes.
• Maintain up-to-date patient health records in EMR and other systems.
• Complete monthly program reporting.
• Attend training as assigned.
