Job summary

Vynca Inc. in California is seeking a Lead Care Manager to coordinate care for individuals with complex needs, collaborating with clinicians, caregivers, and community resources to align services and maximize outcomes.

The LCM will develop care plans, oversee cross-setting coordination, and ensure timely access to benefits using evidence-based approaches and proactive communication. This remote role requires 2+ years in care management, strong communication, and PT alignment.

Qualifications

• Willing to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for evenings/weekends.
• 2+ years experience as a care manager, care navigator, or community health worker serving vulnerable populations.
• Knowledge of government and community resources related to social determinants of health.
• Excellent oral and written communication skills.
• Positive interpersonal skills and teamwork ability.
• General computer skills with Google Workspace, MS Office, and internet.
• Bilingual English/Spanish highly preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative, memory, trauma-informed care, social supports, housing, and community referrals.
• Oversee development of client care plans and goal setting.
• Provide services where members reside or access care (office, telehealth, field-based).
• Link clients to social services and supports per identified needs.
• Advocate with health professionals for client needs.
• Apply evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Conduct outreach and log activity in the CRM system.
• Evaluate progress and update SMART goals; provide mental health promotion.
• Arrange transportation and complete documentation within care plans.
• Maintain up-to-date patient health records and complete monthly reporting.
• Attend training as assigned; perform other duties as needed.
