TrueCare in San Diego and Riverside Counties is seeking an Enhanced Care Management (ECM) Supervisor in California to lead the Care Management team and ensure high-quality patient care.

The role provides guidance, training, and support for center staff, oversees health center questions and conflicts, and monitors productivity and performance while coordinating care for patients with serious or chronic health needs or psychosocial issues.

Competenze

• Associate degree from an accredited institution or equivalent combination of education and relevant care coordination experience.
• Minimum 3 years of care coordination experience in outpatient setting; at least 1 year in enhanced care management.
• Minimum 1 year of experience in a lead capacity.
• Experience in chronic disease management, case management, utilization management, and acute care or Certificate of Completion in Care Management.
• Knowledge of medical terminology and care coordination.
• Knowledge of the HIPAA Privacy and Security Rule.
• Bilingual in English and Spanish.
• Computer proficiency with Electronic Health/Medical Record systems and Microsoft Office.
