Job summary

San Diego Community Health Center is seeking an ECM Supervisor to oversee the Enhanced Care Management program, ensuring whole-person care coordination for members with complex needs. You will supervise the ECM care team, manage caseloads, and serve as the primary operational contact for contracted Medi-Cal plans.

The role emphasizes adherence to CalAIM, DHCS policy guides, and HIPAA; you will drive compliant assessments, care plans, and timely documentation while partnering with hospital

Qualifications

• Bachelor’s degree in social work, public health, nursing, psychology, health administration, or a related field.
• Minimum 3 years of case management, care coordination, or care management experience.
• Minimum 1 year of supervisory, lead, or program coordination experience.
• Working knowledge of Medi-Cal, CalAIM Enhanced Care Management, and the ECM Populations of Focus.

Responsibilities

• Supervise the day-to-day work of ECM staff, including caseloads, outreach lists, and field schedules.
• Monitor staff caseload capacity, productivity, and documentation timeliness; provide coaching as needed.
• Ensure ECM staff complete required training and comply with CalAIM ECM requirements and HIPAA.
