Pacific Health Group is seeking an ECM Program Manager to lead Enhanced Care Management across California counties. You will supervise Lead Care Managers, ensure high-quality care, and drive program growth while advancing health equity and improving outcomes for high-need populations.

This role requires strong leadership, budgeting, and cross-functional collaboration with hospitals, providers, and community organizations to strengthen care transitions.

Qualifikationen

• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Knowledge of Medicaid/Medicare ECM requirements.

• Lead Lead Care Managers across multiple counties to ensure program success.
• Oversee program quality, member services, and cross-functional partnerships.
• Manage budgets, resources, and growth initiatives.
• Coordinate with hospitals and community organizations to improve care transitions.
