Qualifications

• Associate degree or equivalent care coordination experience.
• Min 3 years care coordination in outpatient setting; 1 year in a lead role.
• At least 1 year in a lead capacity.
• Experience in chronic disease management, case management, utilization management.
• Knowledge of HIPAA Privacy and Security Rule.
• Bilingual in English and Spanish.
• Computer proficiency with EMR systems and MS Office.

Responsibilities

• Provide guidance and training to the Care Management team.
• Assist with management of health center questions and conflicts.
• Monitor staff productivity and performance.
• Provide comprehensive care management and care coordination for patients with complex health or psychosocial needs.
