Job summary

Qualifications

• Associate degree or higher in nursing or related field.
• Valid Registered Nurse License.
• At least 1 year of experience in social services, community health, nursing, behavioral health or case management.

Responsibilities

• Provide ECM program oversight and supervise Community Health Workers.
• Develop and implement partnerships and care delivery models for ECM.
• Lead cross-department collaboration to align ECM services with contracts.
• Conduct staff meetings and support performance reviews.
• Identify training gaps and coordinate staff development with Training team.
• Perform patient assessments and create care plans for escalated ECM cases.
• Travel and in-home visits as required.
