Job summary

San Diego Center for Children is seeking a Lead Care Manager (ECM) to coordinate care management for the Enhanced Care Management program. You will oversee case loads, coordinate benefits, and educate families to promote wellness and independence.

You will collaborate with clients, caregivers, and providers, facilitating access to services, and advocating for client needs within a multidisciplinary team.

Qualifications

• Bilingual English/Spanish required; strong communication with clients/families.
• Experience coordinating care across physical/mental health, SUD, trauma-informed care.
• Proficiency with EMR/EHR systems and Care Management tools.
• Strong organization and ability to manage a personal schedule.

Responsibilities

• Assess client needs across health domains including physical/mental health, SUD, trauma-informed care.
• Oversee development and implementation of Individual Care/Health Action Plans.
• Coordinate care and advocate with health professionals to facilitate access to services.
• Attend weekly team meetings and ongoing trainings; participate in data collection and documentation.
